nElara Caring is hiring a compassionate Physical Therapist Assistant to serve home health patients throughout lower Fairfield County.
nThe territory extends from Norwalk to the New York state line , including Stamford and Greenwich .
nAs a Home Health PTA, you will provide personalized therapy in patients' homes under the supervision of a Physical Therapist. You will help patients improve their strength, mobility, balance, safety, and confidence while building meaningful one-on-one relationships.

n nWhat You'll Do
nnnProvide physical therapy treatments according to the established Plan of Care
n
nnWork under the direction and supervision of a licensed Physical Therapist
n
nnHelp patients improve mobility, strength, balance, endurance, and independence
n
nnMonitor patient progress and promptly report changes in condition to the supervising Physical Therapist
n
nnReview the Plan of Care before each visit
n
nnProvide treatment according to physician orders and established clinical standards
n
nnTeach patients and caregivers how to safely complete home exercise programs
n
nnEducate patients on the safe use of walkers, canes, wheelchairs, prosthetics, orthotics, and other assistive devices
n
nnReinforce patient goals and encourage participation in therapy
n
nnComplete accurate and timely clinical documentation
n
nnCollaborate with nurses, therapists, physicians, and other members of the care team
n
n nWhat You'll Bring
nnnGraduate of an accredited Physical Therapist Assistant program
n
nnCurrent and unrestricted Connecticut Physical Therapist Assistant license
n
nnAt least six months of experience as a Physical Therapist Assistant preferred
n
nnStrong communication, organization, and documentation skills
n
nnAbility to work independently in patients' homes
n
nnWillingness to travel throughout Norwalk, Stamford, Greenwich, and the assigned coverage area
n
nnMust possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
n
nnMust be able and willing to travel 50%
n
nnAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
n

nThe base salary for this position is $938.00 to $42.50 per hour, based on the company's good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
