Job description
Physical Therapist Assistant, PTAPutnam Nursing & Rehabilitation Center, is a 160 bed skilled nursing home and rehabilitation facility located in Putnam County, New York. We have maintained the highest regard for our mission to provide high quality care to all individuals with respect and compassion through an interdisciplinary approach that focuses on personalized goals in achieving maximum potential and quality of life.Physical Therapist Assistant, PTA is responsible to organize, and conduct physical therapy programs planned and overseen by the Physical Therapist to facilitate rehabilitation of those residents who have experienced a decrease in functions.Physical Therapist Assistant, PTA will assist in planning, organizing, developing and implementing Physical Therapy services under the supervision of the Physical Therapist, to ensure that the highest degree of quality resident care can be maintained at all times.Physical Therapist Assistant, PTA performs treatment programs for assigned residentsPhysical Therapist Assistant, PTA enters billing information in an accurate and timely mannerReport resident progress in meetings, i.e. UR Meeting, Patient/Family Conference as deemed required or appropriateConsult with other members of the rehabilitation team to coordinate physical therapy with other therapeutic activitiesProvide residents with equipment as appropriateChart notes in an informative and descriptive manner that reflects the therapy provided to the residents, as well as the resident's response to the therapyPerform routine documentation as required and in accordance with our established charting and documentation policies and procedures. Sign and date all entries made in residents' medical record.Cooperate with all departments and staff facilitating harmonious working conditions to promote optimal resident careGraduate of an accredited school of Physical Therapy Assistant, PTALicensed in NY State as a Physical Therapist Assistant, PTA; license in good standingPrevious skilled nursing facility (SNF) experience highly preferredWork well under pressure & willing to work harmoniously with all staffSincere desire to work with the elderly, chronically ill, and handicappedExcellent customer service skillsMust be able to communicate well verbally with residents, families and staffTreat residents and fellow staff with respect, dignity, and compassionCovid-19 vaccinations are encouraged but NOT MANDATED ; we continue to offer COVID-19 vaccination based on eligibility guidance from CDCWe are an Equal Opportunity Employer, celebrate diversity, & are committed to creating an inclusive work environment for all!
