For over 25 years, we've served families across Monterey, San Benito, San Luis Obispo, Santa Barbara, and Santa Cruz Counties, and we're proud to be a two-time Home Health Care Elite Award winner.Our clinicians choose Choice Home Health because of our supportive culture, flexible scheduling, tenured leadership, and genuine sense of family.What You'll DoProvide physical therapy assistance under the direction and supervision of a licensed Physical Therapist (PT)Perform therapeutic interventions designed to improve patient strength, mobility, and functional independenceAssist patients with active and passive therapeutic exercises, muscle re-education, gait and functional training, transfers, prosthetic training, and activities of daily livingUtilize physical agents such as heat, cold, water, light, electricity, ultrasound, and massage, as outlined in the plan of careObserve, document, and report patient treatment, response, and progress to the Physical Therapist, Clinical Manager, and Health Care Provider on a timely basisPromptly report significant changes in patient condition (physical or emotional), safety concerns, unusual occurrences, or adverse effects of medical devicesProvide care and complete documentation in compliance with State, Federal, Medicare, Medi-Cal, and third-party payer regulations and standardsWho You AreCompassionate, dependable, and patient-focusedComfortable working independently in the home settingA team player who values collaboration and supportMotivated by meaningful, one-on-one patient careQualificationsCurrent license as a Physical Therapist Assistant in the state of CaliforniaAt least one year of work experience under the supervision of a licensed physical therapistMust have a vehicle, valid current driver's license and automobile insuranceAbility to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively and submit in a timely manner.Home Health and Home Care Home Base documentation experience preferredSchedule OptionsWe are currently hiring for Full-Time and Per Diem PTA opportunities.
