Our SNF teams work collaboratively with nursing, physicians, and families to deliver meaningful functional outcomes for a diverse geriatric population.As a Physical Therapist Assistant in our Skilled Nursing setting, you will evaluate and treat patients recovering from orthopedic surgeries, neurological events, cardiac conditions, and general deconditioning. You will develop individualized treatment plans, collaborate with the interdisciplinary care team, and play a direct role in helping patients return to their highest level of independence.
