nWhat You’ll Do:

nPerform in-home physical therapy evaluations and develop personalized care plans.

nDeliver skilled therapy services focused on improving strength, balance, gait, and functional mobility.

nEducate patients and family members on home exercise programs and safe mobility practices.

nCollaborate with our interdisciplinary BrightStar team including nurses, OTs, HHAs, and case managers.

nDocument patient progress and visit notes using our mobile-friendly EMR system.

nEnsure patient safety and quality outcomes aligned with BrightStar’s core standards and compliance protocols.

nRequirements:

nActive and unrestricted Massachusetts Physical Therapist license

nGraduate of an accredited PT program (DPT or MPT)

nMinimum 1 year of PT experience preferred (new grads welcome with supervision)

nReliable vehicle and valid driver’s license

nCPR certification

nAbility to work independently and as part of a care team

nFlexible work from home options available.
