About the Role:
As a Physical Therapist (PT) at Avista Nursing and Rehab, you'll be responsible for designing and implementing individualized therapy plans to help patients achieve their maximum potential for independence and well-being. You'll work closely with our interdisciplinary team to ensure that each patient receives comprehensive care that addresses their unique needs and goals.
Responsibilities:
Conduct thorough patient evaluations to identify physical therapy needs and develop personalized treatment plans
Deliver high-quality physical therapy interventions, including exercises, modalities, and other treatments
Collaborate with other healthcare professionals to ensure seamless care coordination and communication
Monitor patient progress, adjust treatment plans as needed, and document patient outcomes
Educate patients and their families on physical therapy techniques, home exercise programs, and other relevant information
Participate in quality improvement initiatives to continuously improve patient care and outcomes
