nPosition Overview Elderwood at Williamsville is seeking a compassionate and dedicatedPhysical Therapistto join our skilled nursing facility team. Our Physical Therapy professionals play a vital role in ensuring the health, safety, and well-being of our residents by providing high-quality, individualized therapy services that support recovery, mobility, and independence.
nResponsibilities
nnnProvide comprehensive physical therapy evaluations and treatments for residents in a skilled nursing setting
n
nnDevelop and implement individualized treatment plans based on resident needs and physician recommendations
n
nnAdminister skilled physical therapy interventions to improve mobility, strength, balance, and overall function
n
nnMonitor resident progress and adjust treatment plans as appropriate
n
nnCollaborate with interdisciplinary team members to ensure coordinated and effective care
n
nnMaintain accurate and timely clinical documentation in accordance with regulatory standards
n
nnEnsure the highest degree of quality care and resident safety is maintained at all times
n
n nQualifications
nnnCurrentState Physical Therapy Licenserequired
n
nnStrong clinical assessment and treatment planning skills

nResponsibilities
nPhysical Therapist:
nnnInitiates skilled physical therapy treatments in accordance with physician’s orders; maintains required contact with physician.
n
nnMaintains written evaluations, progress notes and discharge summaries for individuals in physical therapy program.
n
nnConfers with other health care staff regarding special needs of individuals to enhance rehabilitation or maintenance of function.
n
nnProvides input for meetings and attends during absences of the Director Physical Therapy.
n
nnMaintains statistical records of individuals’ treatments.
n
nnAssists with in-service training programs upon request.
n
nnRecords and updates individual’s plan of care in the Inter-disciplinary Kardex.
n
nnSubmits requisition for supplies needed in the treatment to Director Physical Therapy.
n
nnAssumes all duties of the Director Physical Therapy during absences including supervision of Physical Therapy Aides and responsibility for all necessary documentation.
n
n nQualifications
nPhysicalTherapist :
nnnGraduation from an approved school of physical therapy required.
n
nnState license to practice physical therapy required.
n
nnTwo years’ experience in physical therapy preferred.
n
nnAbility to direct others necessary.
n
nnThis position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
