n nWhat You’ll Do

nAs a Home Health Physical Therapist, you will:

nnnEvaluate patients’ mobility, strength, balance and functional abilities in their homes

n

nnDevelop individualized treatment plans based on each patient’s needs and personal goals

n

nnProvide hands-on therapy to improve movement, safety and independence

n

nnAdjust treatment plans as patients progress and their needs change

n

nnEducate patients and caregivers on exercises, equipment, fall prevention and safe mobility

n

nnCollaborate with physicians, nurses, therapists and other members of the care team

n

nnGuide support staff in helping patients achieve their functional goals

n

nnComplete accurate and timely clinical documentation

n

nnServe as a trusted therapy resource for patients, families, colleagues and the community

n

n nWhat You’ll Need

nnnGraduate of an American Physical Therapy Association-approved physical therapy program

n

nnCurrent, unrestricted Physical Therapist license in the state of practice

n

nnDependable vehicle, valid driver’s license and current automobile insurance

n

nnAbility and willingness to travel throughout the assigned coverage area

n

nnAbility to work independently while remaining connected to a collaborative care team

n

nnSit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.

n
