Seven Healthcare are seeking a dedicated Physical Therapist for a contract assignment in Covington, GA. This position focuses on providing physical therapy evaluation and treatment based on physician referrals within a home health care setting, offering an opportunity to work directly with patients to help them achieve their maximum level of independence.

Key Responsibilities of the Physical Therapist Perform physical therapy evaluation and treatment of patients based upon physician orders/referrals
Complete comprehensive evaluations using appropriate tools and procedures
Review patient records for current diagnosis, history, precautions, and contraindications
Establish measurable goals and develop treatment plans
Regularly reassess the effectiveness of treatment plans and patient progress
Communicate patient needs and progress to the treatment team and others
Consult with medical providers regarding evaluations and treatment
Recommend and assist in securing adaptive equipment for patients
Maintain and submit required documentation
Adhere to standards of business conduct and professional practice
Supervise Physical Therapist Assistants and Aides
Participate in or conduct staff development programs and obtain continuing education
Requirements of the Physical Therapist GA License
Experience in Home Health Care
