Preferred Lymphedema and Oncology experience.
nRequirements:
nnnBachelor's degree; graduate of accredited program
n
nnPhysical Therapist - NH
n
nnCPR certified within 90 days of hire
n
nnPreferred Lymphedema and Oncology experience
n
n nMajor Responsibilities :
nnnSelects; administers and interprets clinically appropriate assessments.
n
nnEffectively integrates educational; medical; social and psychological factors when interpreting evaluative measures and determining the appropriate treatment diagnosis/diagnoses.
n
nnDevelops and implements individual plans of care which promote development and/or restoration of skills required for successful participation in community activities and activities of daily living.
n
nnCollaborates with other healthcare; contract; and school providers to coordinate services and maximize outcomes.
n
nnEvaluates treatment outcomes at various stages and modifies the plan of care to reflect any changes in the patient's performance or condition.
n
nnDesigns home exercise programs to facilitate carry over and promote independence. Provides instruction and supervision in the implementation of home programs.
n
nnComplies with standards for medical; and/or educational; records management (e.g. insurance authorization; consents; physician's referral; documentation of treatment notes) and maintains patient/client confidentiality in accordance with HIPAA guidelines.
n
nnComplies with professional and licensure standards on code of ethics.
n
nnMaintains a safe and therapeutic environment for patient and family in accordance with hospital standards including assurance and support of patient's rights and responsibilities.
n
nnOrients; instructs and directs work activities of assistants; aides and/or students in acute; outpatient and contract settings.

n nPay Range:
n$34.53 - $56.63
nThe pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
