Job description
Physicians Choice Home Health is hiring a full-time or part-time/per diem Physical Therapist (PT) to join our Home Health team, serving patients in the South Bay of Los Angeles. In this role, you will provide high-quality physical therapy services to patients in the comfort of their own homes, helping them regain independence and improve their daily functioning. You will oversee a supporting Physical Therapy Assistant and a team of dedicated healthcare professionals who are committed to delivering exceptional patient-centered care.
nWhat You'll Do
nnnProvide in-home physical therapy evaluations and treatments
n
nnAssess patients’ strength, mobility, balance, and ability to function safely at home
n
nnCreate and update individualized plans of care that support independence and recovery
n
nnHelp patients regain mobility and confidence in their home environment
n
nnEducate patients and caregivers on exercises, safety, and fall prevention
n
nnDocument visits and evaluations in WellSky/Kinnser in a timely manner
n
nnCollaborate with nurses, therapists, and the broader care team to support patient outcomes
n
nnSupervise Physical Therapy Assistants (PTAs) and support their clinical development
n
nnParticipate in care planning, case conferences, and discharge planning
n
n nRequirements
nnnGraduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or DPT)
n
nnActive California Physical Therapist license
n
nnCPR/BLS certification
n
nnStrong communication and organizational skills
n
nnReliable transportation and willingness to travel to patients’ homes
n
nnComfort working independently in a home health setting
n
nnHome health experience preferred
n
nnExperience charting in Kinnser/Wellsky preferred, but not required
n
