Job description

Pride Health is actively hiring a Physical Therapist (PT) to join our client's growing Home Health team.

Compensation:

Local Pay: $55.00 - $70.00/hour (W-2)

Travel Pay Package: Up to $2,800/week gross (includes weekly non-taxable stipend)*

Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Yes

Location: Fort Wayne, IN, 46804

Weekly Hour Guarantee: 36

Coverage Areas: Within 30-40 miles radius

Shift schedule (days, start & end times): Monday to Friday 08:00 AM to 04:30 PM

Key Responsibilities:

Conduct comprehensive home health physical therapy services, including Start of Care (SOC), initial evaluations, routine revisits, re-evaluations, and discharge assessments , while developing individualized treatment plans, improving patients' functional mobility, and meeting a productivity expectation of 30 points .

Deliver evidence-based therapy interventions, monitor and document patient progress accurately in Homecare Homebase (HCHB)/PointCare , collaborate with physicians and the interdisciplinary team, educate patients and caregivers, and ensure compliance with Medicare, agency, and home health documentation standards across the assigned coverage areas.

Must have qualifications/experience: Home health experience preferred.
