nHiring: Physical Therapist (Fort Wayne, IN) – $15,000 Sign-on Bonus
nParkview Health at Home is seeking a full-time or part-time Physical Therapist to join our team in Fort Wayne. Leverage NestMed ambient AI technology to streamline documentation and focus on delivering exceptional patient care.
nSchedule: Mon–Fri, 8:00 AM – 5:00 PM (No after-hours on-call).
nPerks: mileage reimbursement ($0.585/mile).
nIncentives: $15,000 Sign-on bonus.
nWe are seeking a skilled and compassionate Physical Therapist to join Parkview Health at Home team. The successful candidate will provide physical therapy services to patients in their homes, promoting rehabilitation, recovery, and improved quality of life. As a key member of our interdisciplinary team, you will work closely with patients, families, caregivers, and other healthcare professionals to develop and implement personalized treatment plans that address each patient's unique needs and goals.
nThe focus of Physical Therapy is to improve functional movement. Practicing in the home setting, where patients spend most of their time, is the most “functional” place to make an impact on this!
nResponsibilities:
nnnConduct evaluations of patients’ physical abilities, mobility, strength, and functional limitations.
n
nnImplement therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function and mobility.
n
nnEducate patients and their families about exercises, safety, and lifestyle modifications to promote recovery and prevent re-injury.
n
nnMonitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
n
nnMaintain accurate and up-to-date patient records in compliance with state regulations and agency policies.
n
n nJob Requirements
nnnBachelor’s degree from an accredited school approved by the American Physical Therapy Association
n
nnActive registered PT license in the state which you practice
n
nnMust obtain and maintain current CPR/BLS Certification
n
nnOne-year physical therapy experience in teaching or clinical setting is preferred, but not required.

nnCurrent Drivers License
n
nnComply with federal, state and local laws and regulations
