nBAYADA Home Health is seeking a compassionate and experienced Physical Therapist (PT) to provide one-on-one home health visits to adult and senior clients throughout Monmouth, Somerset, and Union Counties, New Jersey.

n nResponsibilities
nAs a Home Health Physical Therapist, you will:
nnnProvide skilled physical therapy services to adult and geriatric patients in their homes
n
nnComplete comprehensive physical therapy evaluations, including assessments of:
n
nnJoint mobility and range of motion
n
nnMuscle strength and endurance
n
nnBalance, gait, and functional mobility
n
nnTransfers and ambulation
n
nnHome safety and fall risk
n
nnDevelop and implement individualized treatment plans designed to maximize function, mobility, and independence
n
nnProvide therapeutic exercise, manual therapy, gait training, balance training, and other evidence-based interventions
n
nnImplement appropriate interventions to promote skin integrity and support wound prevention and healing
n
nnCollaborate with nursing and the interdisciplinary care team to coordinate patient care
n
nnEducate patients and caregivers on home exercise programs, fall prevention, safe mobility techniques, and equipment use
n
nnReassess patient progress and modify treatment plans as appropriate
n
nnComplete accurate and timely documentation using an electronic medical record (EMR) system
n
n nQualifications
nnnCurrent Physical Therapist (PT) license in the State of New Jersey
n
nnMinimum of one year of recent Physical Therapist experience
n
nnGraduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA) or equivalent accrediting body
n
nnHome Health experience preferred, but not required
n
nnStrong clinical assessment, communication, organizational, and critical thinking skills
n
nnComfortable using electronic medical records (EMR)
n
nnAbility to work independently while collaborating with an interdisciplinary care team
n
nnReliable transportation and willingness to travel throughout Monmouth, Somerset, and Union Counties
n
n nAdditional Information
nBase Pay, depending upon qualifications, status, and subject to negotiation:
nFull-Time
n$75.00-$80.00 per point ($117,000- $124,800 annual earning potential)
nPer Diem
n$75.00-$80.00 per point
n$10,000 Sign-On Bonus for Full-Time PTs Only
