Position OverviewHome Health Physical Therapist Signature Healthcare at Home is hiring a full‑time Physical Therapist to serve the Westside Portland Metropolitan Area, including neighborhoods such as Beaverton, Tigard and surrounding communities in the SW/NW region. The NE Portland team is known for its strong communication, supportive leadership, and clinician‑centered environment.Key ResponsibilitiesPT ResponsibilitiesConduct home‑based physical therapy evaluations.Develop individualized treatment plans focused on functional mobility and independence.Deliver therapeutic interventions including gait training, strengthening, balance work, and home‑exercise program development.Perform home‑safety assessments and recommend appropriate DME.Manage a field‑based caseload with efficiency and autonomy.Collaborate with interdisciplinary team members to ensure coordinated care.Maintain accurate documentation in accordance with regulatory standards.Support safe transitions from hospitals, SNFs, and outpatient settings.ScheduleFull‑Time ScheduleMonday–FridayNormal business hoursStrong interdisciplinary supportQualificationsPT RequirementsCurrent Oregon Physical Therapist license (or ability to obtain).One year of PT experience preferred; home‑health experience a plus.Strong communication, assessment, and clinical reasoning skills.Ability to work independently in a field‑based environment.Reliable transportation and valid driver’s license.Compensation & BenefitsCompensation DetailsCompetitive salary or hourly payComprehensive benefits packageMileage reimbursementPaid time off and paid holidaysContinuing education supportOpportunities for professional growth within Signature and The Pennant GroupService AreaNE Portland Metro This role serves the Northeast Portland Metro region, offering clinicians a diverse caseload and the opportunity to build meaningful patient relationships across vibrant neighborhoods.The employer for this position is stated in the job posting.
