Physical Therapist (PT)

Full-Time | Part-Time | PRN

? $10,000 Sign-On Bonus for Full-Time Positions

We are seeking a licensed, compassionate, and patient-focused Physical Therapist (PT) to join our rehabilitation team and provide high-quality, skilled therapy services to residents in accordance with physician orders and individualized care plans.

This is an excellent opportunity for a therapist who is passionate about clinical excellence, patient-centered care, and helping residents improve mobility, function, independence, and overall quality of life .

Key Responsibilities

Evaluate residents and develop individualized physical therapy treatment plans based on clinical needs and physician orders.

Provide skilled physical therapy services in accordance with established care plans.

Monitor resident progress and modify treatment approaches as clinically appropriate.

Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge summaries.

Collaborate with nursing, therapy, and interdisciplinary team members to support positive resident outcomes.

Participate in care planning, utilization review meetings, interdisciplinary conferences, and other clinical meetings as assigned.

Educate residents, families, and staff regarding therapy goals, treatment plans, safety, mobility, and functional strategies.

Maintain established productivity, documentation, and quality standards.

Support regulatory compliance, reimbursement requirements, and survey readiness.

Promote resident safety, dignity, independence, mobility, and overall well-being.

Qualifications

Graduate of an accredited Physical Therapy program.

Current and active state license as a Physical Therapist.

Long-term care or skilled nursing experience preferred.

Strong clinical documentation and communication skills.

Excellent interpersonal, organizational, and time-management skills.

Ability to work effectively within an interdisciplinary healthcare team.

Compassionate, patient-focused, and committed to providing high-quality care.

Physical Requirements

Ability to stand, walk, bend, and assist residents throughout the workday.

Ability to safely lift, transfer, and support residents during therapy activities.

Ability to perform physical tasks associated with patient care and therapeutic treatment.

Comfortable working in a clinical and long-term care environment.

Ability to manage the physical and emotional demands of patient care.
