Job description
Chatham Hills Subacute Care Center is seeking an Experienced Physical Therapist (PT) to evaluate and treat patients in our subacute facility. The PT will assess rehabilitation needs, design individualized plans, and educate patients and families while coordinating with the care team.
Responsibilities include supervising PT Assistants, maintaining thorough documentation, and ensuring HIPAA compliance. Prior experience in long-term or geriatric care is preferred.
