Job description
The Chatham Hills Subacute Care Center is seeking an experienced Physical Therapist to join our rehabilitation team. The PT will assess referred patients, develop and manage treatment plans, and document patient progress under physician direction.
You will supervise Physical Therapy Assistants, collaborate with all disciplines, educate patients and families, and ensure confidentiality and compliance with HIPAA and residents' rights. Prior geriatric and long-term care experience preferred.
