We are seeking a compassionate and skilled Physical Therapist to join our rehabilitation team at Serenity Spring Senior Living at Green Bay. As a Physical Therapist at our senior living community, you will have the opportunity to work with a diverse population of older adults, providing exceptional care and rehabilitation services to help them achieve their maximum level of independence and well-being.
Summary of Responsibilities:
Design and implement individualized physical therapy treatment plans to meet the unique needs of our residents
Conduct thorough evaluations and assessments to identify areas of strength and weakness, setting goals for improvement and progress
Provide one-on-one therapy sessions, utilizing a variety of techniques and modalities to promote healing and recovery
Collaborate with other members of the healthcare team to ensure comprehensive and coordinated care
Maintain accurate and timely documentation of patient progress and treatment plans
Participate in quality improvement initiatives to enhance the overall quality of care and services provided
