nJob Summary
nA Physical Therapist provides services to patients who have impairments, functional limitations or changes in physical function from injury, disease or other causes. A Physical Therapist performs evaluations and establishes plans of care for patients based upon age specific considerations and patient/family goals in various treatment arenas. The physical therapist continually assesses the treatment program and patient's discharge planning needs, recommending changes to the program as appropriate. Supervises physical therapist assistants or rehab techs as assigned, participates actively in work process and program development to enhance the work environment, and participates in other duties as assigned. Utilizes time efficiently and effectively. Adheres to scheduling and productivity guidelines. Staff schedules require flexibility as they are dependent on patient care needs, which may include weekend and evening hours as necessary, to meet demand in their primary are or the needs across the system. Promotes and attend community outreach events as necessary.

nnDemonstrates appropriate use of various treatment modalities in patient care.
n
nnAdheres to ethical and legal practice standards.
n
nnAbides by relevant ethical codes and standards of practice guidelines.
n
nnAbides by State and Federal laws and regulations, including state licensure laws.
n
nnReports any concerns to rehab leadership, or compliance line as appropriate.
n
nnMaintains records that are accurate, concise, timely and legible in accordance with System, IT or Payor policies.
n
nnDocuments therapy care in format and accordance with practice setting on EMR including: evaluations, treatment provided, response to treatment, patient progress, education, family conferences, discharge planning and discharge summary as appropriate.
n
nnCompletes reassessments at appropriate intervals and documents accordingly.
n
nnCommunicates post-discharge needs to appropriate resources.
n
nnDocuments patient care that is consistent with guidelines and requirements of agencies and third party payers.
n
nnEducates all patients and families in a manner consistent with their level of understanding and therapeutic needs.
n
nnInvolves patient and family members in establishing a plan of care.
n
nnProvides patient and family members with educational materials as needed.
n
nnDocuments education and training as well as patient and/or family level of achievement with training provided.
n
nnCaptures maximum revenue for quality services provided while maintaining established standards of care.
n
nnUtilizes appropriate documentation including CPT and CDM codes to match services provided.
n
nnMaintains adherence to established efficiency standards.
n
nnChecking for accuracy in charging based on therapeutic intervention provided prior to signing and finalizing daily documentation.
n
nnProvides relevant information to insurance provider to insure proper authorization is secured prior to initiation of treatment (when indicated).
n
nnAssures treatment does not extend beyond last covered date.
n
nnPromotes continual performance improvement both within the Rehabilitation department and throughout Independence Health.
n
nnDemonstrates individual initiative for performance improvement both within the department and throughout Independence Health.
n
nnActively identifies needs for improvement in the department and system wide and discusses with manager.
n
nnCollaborates with patients, families, and other health team members to provide excellence in patient care.
n
nnCommunicates regularly with other team members via verbal, written, and/or established electronic communication methods.
n
nnAttends interdisciplinary team meetings when indicated.
n
nnAttends all required staff meetings. If absent or unable to attend, reads minutes and signs appropriate sign-in sheets to indicate review of material.

nnParticipates actively in the ongoing development of new therapy programs and in the enhancement of established programs.

nnAttends appropriate internal or external education opportunities to promote growth and expansion of specific programs

nnResearches appropriate innovative ides via journal and internet exploration and discusses these with manager leads, and peers.

n
nnDemonstrates an awareness of strengths and weaknesses and seeks guidance as needed to address limitations.

nnInitiates and participates in continuing education: lectures, video/audio conferences, written and online courses.

nnDevelops and conducts in-service as appropriate to share information gained from external resources.

nnUtilizes supplies and equipment in a fiscally responsible manner.

nnTravel to other entities or sites within the System as patient census dictates, in order to provide quality necessary care where patient need is greatest.

nnStaff schedules require flexibility as they are dependent on patient care needs, which may include evening and weekend hours, as necessary to meet demand in their primary area or the needs across the System.
n
nnUnderstand and facilitate obtaining authorizations in conjunction with revenue specialist.
n
nnKnowledge & compliance with documentation of G codes as mandated by insurance.
n
nnUnderstanding & working knowledge of insurances and their documentation, authorization and billing framework and requirements.
n
nnCommunicates to lead/leadership any changes or volume in census that will impact daily productivity of the clinic.
n
nnParticipates in supervision of students as appropriate.
n
nnOther duties as assigned.

n nRequired Qualifications
nnnBachelor’s in Physical Therapy or higher level of education.
n
nnStrong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment.
n
nnStrong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.
n
nnStrong time management and ability to work independently and in a team setting; creativity in treatment in various settings; ability to transition smoothly between various treatment settings; flexibility in scheduling.
n
nnDemonstrate understanding of current billing requirements and adherence to payor guidelines.
n
n nPreferred Qualifications
nnnKnowledge and Application of Microsoft Office software.
n
nnKnowledge of Internet and appropriate usage; Research experience
n
nnPrevious clinical experience/knowledge and/or certification in specialty treatment programs including, Lymphedema, Hand Therapy, Aquatics, Occupational Medicine, Concussive/vestibular, and/or Sports Medicine, Women’s Health, Neuro.
n
nn1-3 years’ experience in healthcare setting.
n

n nLicense, Certification & Clearances
nnnCurrent Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program.

nnAct 33 with renewal

nnCurrent license as a Physical Therapist in Pennsylvania or eligible for licensure in State of Pennsylvania

nnGraduate Therapist is expected to sit for the licensure exam at the soonest availability, not to exceed six (6) months following date of hire.

n nSupervisory Responsibilities
nnThis position has no direct supervisory responsibilities, but does serve as a coach and mentor for other positions in the department.

nnnIncumbent will be scheduled based on operational need (rotate shifts, standby, on-call, etc.).
