What You'll Do:
Provide high-quality physical therapy services to patients in their homes.
Evaluate patients' physical conditions and develop personalized treatment plans.
Work collaboratively with a team of healthcare professionals to ensure comprehensive care.
Educate patients and their families on therapy techniques and lifestyle changes.
Monitor progress and adjust treatment plans as needed to achieve optimal outcomes.
Qualifications:
Current state licensure as a Physical Therapist.
Proven experience in a home health setting is preferred but not required.
Strong interpersonal and communication skills.
Ability to work independently and manage time effectively.
Valid drivers license and reliable transportation.
