Overview
Our Visiting Physical Therapist for our VNA Certified division is responsible and accountable for evaluation, coordination, and administration of quality physical therapy services.
At Aveanna, our services include Skilled Nursing, Physical Therapy, Occupational Therapy, and Home Health Aide support delivered in patients' homes. We serve diverse populations across all ages, including patients with chronic conditions, behavioral health needs, skilled nursing requirements, and therapy needs.
Pay
$65 per hour/visit based on the pay point model

Newly hired Variable Hour and Part Time employees are measured for eligibility into our ACA Benefits plans over their first twelve months of employment (also known as the Initial Measurement Period). What You'll Do Delivery of physical therapy services to comply with prescribed physician orders.
Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws.
Provide a written evaluation defining physical therapy needs, goals, progress and expected outcomes in conjunction with referring physician's Plan of Treatment.
Participates in coordination of services (including discharge planning) with health care team, physicians, parent/guardians, nurses, etc.
Maintain effective communication between staff, healthcare team members and family.
Serve as a resource consultant for clinical staff.
Supervises other personnel (PTA, HHA) as applicable.
Completes OASIS assessments where allowed by state professional practice and regulation.
Qualifications A certification in Physical Therapy in the state of application
Current and unrestricted license in the state of practice
Current CPR certification (with hands-on component) or able to obtain one
Current TB test or able to obtain one
Must have a drivers license and reliable transportation

Responsibilities & Qualifications
Qualifications Must have and maintain an active, unencumbered license (PT) in the state in which the clinician will practice
Compact licenses must be transferred to your state of residence within 90 days
Current CPR certification (with hands-on component)
TB skin test (current within last 12 months)
Six months prior hands-on experience preferred but not required
Must have reliable transportation
