n nWhat You’ll Do as a Home Health Physical Therapist:
nnnDeliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.
n
nnEvaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.
n
nnAdjust treatment plans and techniques as needed to achieve optimal outcomes.
n
nnCollaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care.
n
nnEducate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.
n
nnDocument all visits, assessments, and progress notes accurately and in a timely manner.
n
nnSupport patients in maximizing their independence and improving quality of life—wherever they call home.

n nAbout You
nQualifications – What You’ll Bring:
nnnMaster’s degree in Physical Therapy from an accredited program.
n
nnCurrent PT license in the state of employment (or eligibility to obtain).
n
nnCPR certification required.
n
nnStrong communication, assessment, and problem-solving skills.
n
nnAbility to work independently in a variety of home environments.
n
nnValid driver’s license, auto insurance, and reliable transportation.
n
n nPreferred Experience (Not Required):
nnn1+ year of experience in a hospital, home health, or outpatient rehab setting
n
nnIn-home therapy delivery
n
nnGeriatric rehab, orthopedic or neurologic rehab
n
nnPost-acute care or chronic condition management
n
nnWorking as part of a mobile or interdisciplinary care team
n
