Job description
VNS Health is seeking a compassionate Physical Therapist, Home Care to provide high-quality, patient-centered services across New York City. In this community-based role, you will conduct in-home assessments, create individualized treatment plans, and deliver therapeutic interventions that restore mobility, reduce pain, and improve safety and independence. You will collaborate closely with an interdisciplinary team, support caregivers through education, and document outcomes using EMR systems.
