nJob Description:
nWe are looking for a dedicated Home Care Physical Therapist to join our compassionate team in providing high-quality care to individuals in the comfort of their homes. As a Home Care Physical Therapist, you will play a crucial role in helping clients regain mobility, manage pain, and improve their overall well-being.
nKey Responsibilities:
nn Conduct comprehensive assessments of clients' physical conditions in a home care setting.
n Develop personalized and goal-oriented physical therapy plans based on assessment findings.
n Administer therapeutic exercises, manual therapy, and other interventions to enhance clients' functional abilities.
n Collaborate with clients, their families, and other healthcare professionals to ensure coordinated care.
n Educate clients on proper techniques for home exercises and self-care.
n Maintain accurate and thorough documentation of assessments, interventions, and progress.
n nQualifications:
nn Degree in Physical Therapy from an accredited program.
n State licensure as a Physical Therapist.
n Strong clinical and interpersonal skills.
n Ability to adapt therapy plans to meet the unique needs of home care clients.
n Compassionate and patient-centered approach to care.
n nExperience:
nPrevious experience in home care or a related field is preferred. New graduates with a passion for home care are encouraged to apply.
