Job Description:

Responsibilities: Assess patients' physical abilities and develop personalized treatment plans in the home setting

Provide therapy to improve mobility, strength, and functional independence

Collaborate with patients, families, and caregivers to support rehabilitation goals

Educate patients and families on exercise programs and safety measures

Monitor and document patient progress and adjust treatment plans as needed

Implement interventions tailored to the home environment and lifestyle

Coordinate care with healthcare teams and community resources for comprehensive support

Qualifications: Doctorate in Physical Therapy

VA Physical Therapist PT license

Current BLS/CPR Certification

Clean Driving Record

Job details: Pay: $2,467.23 - $2,543.54 / week

Location: Falls Church, VA

Assignment Length: 13

Shift: Day - up to 40.00 hours per week

Setting: Home Health

Workplace: On-site
