Physical Therapist Home Health, La Jolla
Hourly Pay Rate: $48-$75

Cheer Home & Health Care, Inc. is hiring a Physical Therapist in San Diego. The Physical Therapist responsibilities will include:
Provide evaluations, treatments, and education per Plan of Care
Perform initial and ongoing patient assessments
Develop and update treatment plans with care team
Notify physicians of patient condition changes
Document care notes promptly (within 24 hours)
Establish therapy goals and deliver appropriate interventions
Ensure patient safety, infection control, and proper equipment use
Train patients/caregivers on therapy and equipment use
Assess home environment safety and recommend solutions
Maintain confidentiality and professional conduct
Requirements Active California PT license
3 years of experience
Industry Experience, preferred
Upbeat personality
Strong Computer Skills
Ability to multitask, drive, take action
Strong Organizational & Communication Skills
Pass Live Scan Fingerprint Clearance
Valid driver's license and proof of insurance required
Cell phone with Internet Access
Proof of Negative TB results
