Physical Therapist (Home Health)
Location: Tracy, Stockton, and Manteca, CA
Schedule: Full-Time
Compensation: Per-Visit Pay Structure
We’re seeking a compassionate and motivated Physical Therapist to join our growing team of clinicians dedicated to delivering exceptional, patient-centered care in the comfort of patients’ homes. What You’ll Do
Conduct initial patient visits, including comprehensive assessments and evaluations
Develop and implement individualized treatment plans
Supervise and collaborate with Physical Therapist Assistants (PTAs)
Coordinate care with interdisciplinary team members

What You Bring
Current, active Physical Therapist license in the state of employment
Valid driver’s license, auto insurance, and reliable transportation
Current BCLS/CPR certification
Home health experience preferred, but not required
Ability to pass background check, drug/alcohol screening, and OIG screening
