Job description
NOW HIRING a Physical Therapist for per diem opportunities in a home care setting. You will be paid for each completed patient visit (with documentation).

Responsibilities
Perform home visits for patients in a home care setting
Observe, record, and coordinate care with multi-disciplinary team regarding the patient's treatment and any change in condition
Teach, supervise, and counsel the patient and family regarding home therapy procedures and other care needs as appropriate to the patient’s condition
Qualifications
Current license as a Physical Therapist in Connecticut
Valid driver’s license
Reliable transportation and willingness to travel in New London County
Strong communication and interpersonal skills
Home health experience including Medicare preferred
