About the Opportunity
We're seeking an experienced Physical Therapist (PT) for a permanent Home Health opportunity serving patients throughout the West Valley of Phoenix, Arizona .
This position offers an excellent combination of competitive compensation, independence, and meaningful one-on-one patient care. You'll work directly with patients in their homes to improve mobility, strength, safety, and overall independence while collaborating with a supportive interdisciplinary team.
Responsibilities
Perform comprehensive in-home Physical Therapy evaluations and reassessments.
Develop and implement individualized plans of care.
Provide therapeutic exercise, gait, balance, mobility, and functional training.
Help patients improve safety and independence in their home environment.
Educate patients and caregivers on home programs, fall prevention, and safety.
Communicate patient progress and changes with physicians and interdisciplinary team members.
Maintain accurate and timely clinical documentation.
Effectively manage an assigned Home Health territory and caseload.
Qualifications
Graduate of an accredited Physical Therapy program.
Active Arizona Physical Therapist license in good standing.
Home Health experience required.
Current BLS/CPR certification.
Valid driver's license and reliable transportation.
Strong clinical judgment and ability to work independently.
Excellent communication, organization, time-management, and documentation skills.
