Job description
Experienced Home Health Physical Therapist (PT)
One Home Health serves patients throughout Porter County and surrounding communities in Northwest Indiana. Multiple service territories are available, and assignments are made based on patient needs, clinician preference, and geographic location whenever possible.

Compensation
Job Type: Part-Time, PRN/Per Diem Pay: $60.00 - $80.00 per hour Location: Porter County and Surrounding Northwest Indiana Communities
Compensation is based on experience, qualifications, and clinical expertise.

Responsibilities
Perform comprehensive physical therapy evaluations in patients' homes.
Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
Provide skilled physical therapy interventions according to physician orders and established care plans.
Assess patients' home environments and recommend safety modifications and assistive devices as appropriate.
Educate patients and caregivers on exercises, injury prevention, fall prevention, and home safety.
Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary care team.
Monitor patient progress and adjust treatment plans based on clinical findings.
Complete accurate, timely, and compliant clinical documentation.
Participate in discharge planning and patient outcome evaluations.
Qualifications
Required
Active Illinois Physical Therapist (PT) license.
Graduate of an accredited Physical Therapy program.
Current CPR/BLS certification.
Valid driver's license and reliable transportation.
Excellent communication, organization, and time-management skills.
Ability to work independently in a home health setting.
Preferred
Home health experience preferred.
Experience with Medicare home health regulations and documentation requirements.
Knowledge of OASIS documentation.
Experience with Axxess software.
Strong orthopedic, neurological, geriatric, or rehabilitation background.
Bilingual candidates are encouraged to apply.
