Job Description
We are seeking a Physical Therapist for a home health setting. The PT will provide in‑home therapy services, develop individualized treatment plans, and collaborate with interdisciplinary care teams to support patient recovery and independence.
Requirements
Active Physical Therapist license
Home health experience preferred
Strong documentation and time management skills
