Physical Therapist Job Requirements Minimum of 1 year of experience as a Physical Therapist required
Current Physical Therapist license in the applicable state
Experience in homecare, home health, or community-based patient care preferred
Strong assessment, treatment planning, and patient education skills
Ability to work independently and manage a mobile patient caseload
Excellent communication, documentation, and care coordination skills
Responsibilities Evaluate patients in the home setting and develop individualized physical therapy treatment plans
Provide therapeutic interventions to improve strength, mobility, balance, and functional independence
Educate patients and caregivers on exercises, safety strategies, and ongoing care plans
Document patient progress, treatment outcomes, and care coordination activities accurately and timely
Collaborate with interdisciplinary team members to support continuity of care
Adjust treatment plans based on patient response and evolving clinical needs
