Home Health Physical Therapist (PT)
Morgan City, LA and Surrounding Areas
RATE: $53.50 per hour
Are you a compassionate and experienced Physical Therapist looking to make a meaningful impact in patients' lives? We are seeking a dedicated Home Health Physical Therapist (PT) to join our team and provide exceptional in-home care throughout the Morgan City, Louisiana region.
Position Overview
As a Home Health Physical Therapist, you will evaluate patients in their homes, develop individualized treatment plans, and provide skilled therapy services designed to improve mobility, function, and quality of life. This role offers the opportunity to work independently while making a direct difference in the communities we serve.
Schedule Full-Time: 40 hours per week
Monday through Friday
8:00 AM to 4:30 PM
Productivity Expectations 30 productivity units per week
Coverage Area
The service territory includes Morgan City and surrounding communities, including: Jeanerette (westernmost coverage area)
Charenton
Baldwin
Franklin
Centerville
Patterson
Berwick
Morgan City
Stephensville
Pierre Part
Amelia
Bayou Lourse
Portions of Gibson (including Bayou Black Road up to the school)
Key Responsibilities Perform comprehensive home health physical therapy evaluations
Develop and implement individualized Plans of Care
Provide skilled physical therapy treatment in the home setting
Complete OASIS assessments and required clinical documentation
Coordinate care with physicians, nursing staff, caregivers, and interdisciplinary team members
Monitor patient progress and adjust treatment plans as needed
Ensure compliance with all applicable home health regulations and agency policies
Required Qualifications Current Louisiana Physical Therapist (PT) License
Home Health Physical Therapy experience
Experience using Homecare Homebase (HCHB)
Strong clinical assessment and documentation skills
Ability to work independently and manage a flexible territory
Preferred Qualifications Extensive Home Health experience
Advanced proficiency with Homecare Homebase (HCHB)
Excellent communication and patient education skills
Strong organizational and time-management abilities
Additional Information Mileage reimbursement provided between patient visits
Occasional availability for weekend or on-call evaluations preferred
Professional attire required; navy scrubs preferred, but any clean, professional color is acceptable
