Job description
Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Erie, PA. This role supports high-quality resident care through evaluation and treatment planning.
Responsibilities include delivering PT services per standards, documenting and CMS requirements in NetHealth EMR, and collaborating with an interdisciplinary team to reach goals. A BS in Physical Therapy and active license are required, with ongoing CEU encouraged.
