Evolve Therapy Services is seeking a dedicated Physical Therapist to join our in-house Skilled Nursing Facility team in Queens Village, NY. Provide comprehensive PT services, assess residents, and develop treatment plans within CMS and NetHealth EMR guidelines.
Requires active state license, BS in Physical Therapy, and commitment to ongoing professional development. Full time, part time, or PRN options available.
