Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in South Sioux City, NE. This full-time role (also options for Part Time or PRN) includes a $10,000 sign-on bonus and requires applying lifelong PT standards to ensure high-quality resident care.
Responsibilities include evaluating residents, developing individualized treatment plans, documenting per CMS guidelines, and collaborating with the interdisciplinary team to improve function and mobility.
