Position Overview
We are seeking a motivated and compassionate Outpatient Physical Therapist to join an independently owned physical therapy practice in Fort Wayne, Indiana.
This is an excellent opportunity for a Physical Therapist who values high-quality, individualized patient care and wants to work in a supportive environment where clinicians have the time and resources to focus on their patients.

Key Responsibilities Evaluate patients and develop individualized treatment plans based on their specific needs and goals.
Provide one-on-one physical therapy treatments using evidence-based techniques.
Monitor patient progress and modify treatment plans as appropriate.
Educate patients and families on exercises, home programs, injury prevention, and recovery.
Maintain accurate and timely clinical documentation.
Collaborate with other members of the therapy and healthcare team.
Build strong relationships with patients while providing compassionate, high-quality care.
Participate in ongoing professional development and mentorship opportunities.
Qualifications PT or DPT from an accredited program
Active Indiana Physical Therapist license
Strong communication and interpersonal skills
Team-oriented personality
Commitment to patient-centered, high-quality care
Ability to work independently while contributing to a collaborative team
