Phoenix of Maple Heights is hiring a Full-Time Physical Therapist (PT) to join our dedicated rehabilitation team in Maple Heights, Ohio!

What You'll Do
Evaluate residents and develop individualized treatment plans
Provide skilled physical therapy services in a SNF/LTC setting
Help residents improve strength, balance, mobility, transfers, and functional independence
Monitor progress and modify treatment plans as needed
Collaborate with nursing, therapy, physicians, and the interdisciplinary care team
Participate in care planning and discharge planning
Complete accurate and timely documentation using Net Health and PointClickCare (PCC)
Provide compassionate, resident-centered care
What We're Looking For
Graduate of an accredited Physical Therapy program
Current or eligible Ohio Physical Therapist license
Strong clinical, communication, and interpersonal skills
Team-oriented approach to resident care
SNF/LTC experience is a plus
