This role specializes in providing high-quality care to individuals in their homes or alternative settings. The successful candidate will play a pivotal role in patient rehabilitation, following physician-approved plans of care.

Key Responsibilities of the Physical Therapist Evaluate patient needs and develop individualized treatment programs
Administer complex therapeutic exercises
Instruct, motivate, and assist patients and caregivers in improving functional mobility
Provide clinical supervision and oversight of Physical Therapist Assistants and Home Health Aides
Ensure appropriate use of adaptive equipment
Maintain effective communication with the interdisciplinary care team
Requirements of the Physical Therapist Valid Physical Therapy license in NC
Proficiency in Physical Therapy and Oasis Assessment
Experience with EMR - HCHB
