nEnhance Rehabilitation is currently seeking qualified Physical Therapists candidates to join our dynamic team. As an Enhance Rehabilitation Physical Therapist, you will be a part of growing team providing the highest quality care for patients in SNF setting. In this role, you will evaluate and treat patients following an individualized plan of care.
nnnProviding excellent care for our patients

nnProviding excellent customer service to our patients and family members
n
nnCollaborate with facility staff to ensure excellent care for you patients
n
nnEvaluating new patients, developing plans of care and setting measurable, functional goals
n
nnTreating patients, answering their concerns and educating them on their injury, progress and probable outcomes
n
nnSupervising Physical Therapist Assistants
n
nnRe-evaluating patient progress, revising plans of care and resetting goals
n
nnComplete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
