We provide a wide variety of out-patient physical therapy services including sports and orthopedics, vestibular/concussion, pelvic health, pediatrics, athletic training outreach services, wellness plans, etc.

nWe are seeking a dedicated Physical Therapist to join our healthcare team. As a vital member of our rehabilitation services, you will provide comprehensive physical therapy care while working collaboratively with our multidisciplinary team to deliver optimal patient outcomes.
nnnConduct thorough patient evaluations and develop individualized treatment plans
n
nnImplement evidence-based physical therapy interventions and monitor patient progress
n
nnDocument patient assessments, treatment plans, and progress notes with attention to detail
n
nnSupervise and coordinate care delivered by physical therapy assistants and rehabilitation technicians
n
nnEstablish measurable treatment goals and adjust plans based on patient progress
n
nnProvide patient and caregiver education regarding treatment plans and home exercise programs
n
nnCollaborate with referring physicians and other healthcare providers
n
nnMaintain accurate and timely documentation in compliance with healthcare standards
n
nnUtilize rehabilitation equipment and techniques effectively
n
nnParticipate in regular team meetings and care coordination discussions
n
n nQualifications
nnnGraduate degree from a CAPTE-accredited Physical Therapy program
n
nnCurrent Minnesota state physical therapy license or eligibility to obtain one
n
nnValid CPR certification
n
nnStrong knowledge of anatomy, physiology, and rehabilitation principles
n
nnExcellent assessment and diagnostic skills
n
nnProven ability to develop and implement effective treatment plans
n
nnStrong documentation and electronic health record skills
n
nnOutstanding interpersonal and communication abilities
n
nnPhysical stamina and ability to assist patients with mobility
n
nnProblem-solving and critical thinking skills
n
nnAbility to work independently and as part of a healthcare team
n
nnCommitment to continuing education and professional development
n
nnExperience with diverse patient populations preferred
n

n nAdditional Information
nThe salary range begins at $72,000 and increases based on relevant experience. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Additionally, this position is eligible for discretionary incentive compensation. The Company's incentive compensation plan is subject to change. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan
