Job description
Provide one-on-one physical therapy to older adults in their homes and senior living communities. Evaluate patients, develop individualized care plans, deliver evidence-based treatment, and collaborate with an interdisciplinary team to support mobility, independence, and quality of life. Telehealth care may also be provided when appropriate.
