n nResponsibilities
nWhat you will do: Responsibilities listed include but not limited to:
nnnPerform evaluations and develop treatment plans
n
nnRegularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
n
nnPerform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
n
nnCommunicate patient’s needs and progress to the treatment team, physician, person receiving services and family members
n
nnCommunicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes
n
n nQualifications
nWhat you will need:
nnnMinimum of bachelor’s degree in Physical Therapy from a college or university with accredited Physical Therapy program
n
nnCurrent, unrestricted license as Physical Therapist by state in which practicing
n
nnCurrent Basic Life Support or Cardiopulmonary Resuscitation certification in accordance state regulations
n
nnA minimum of one year’s work experience as a physical therapist preferred
n
nnDemonstrates knowledge of rehabilitation techniques related to complex neurological injury
n
nnCommunicates effectively and professionally in verbal and written interactions
n
nnAbility to lift 50 pounds. Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
n
nnDuties require fine motor skills , visual acuity, and walking/ standing for extended periods
n
nnAdditional physical requirements include Pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
n
