nMonday-Friday including some day and evening hours,
nOne Saturday per month.
nGeneral Summary
nProvides physical therapy services at an entry to intermediate skills level to individual patients according to the principles and practices of physical therapy and works collaboratively with interdisciplinary treatment team.
nResponsibilities
nDuties and Responsibilities
nEssential Functions:
nnnEvaluates patient using diagnostic and prognostic muscle, nerve, joint and functional ability tests.
n
nnPlans individualized physical therapy program for each patient including exercises, therapeutic modalities and procedures.
n
nnDirects and supervises patient in active, active assisted, passive exercises, muscle re-education, and functional training using appropriate equipment as needed.
n
nnProvides instructions in therapeutic procedures to be continued by the patient.
n
nnCoordinates physical therapy activities in collaboration with the other members of the interdisciplinary team to facilitate achievement of goals established.
n
nnConducts or participates in training medical and nursing personnel in physical therapy techniques and objectives.
n
nnUtilizes safe patient handling techniques for all patient transfers, lifts and positioning.
n
nnProvides supervision of PTAs and Rehab Aides as required by PA practice act.
n
n nCommon Expectations:
nnnDemonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).

nnMaintains patient record, as required by profession, with accurate documentation to support medical necessity and functional abilities.
n
nnEnsures the process design of established policies/procedures, objectives, quality management, safety, environment and infection control standards are utilized consistently.
n
nnEnhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
n
nnActs as a role model in Red Rules compliance with a focus on hand hygiene and patient identifiers.
n
nnCollaborates with other team members to create a culture of patient centeredness and clinical excellence.
n
nnUtilizes SBAR as a standard method of communication in all aspects of patient care.
n
nnProvides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.

nMinimum Education:
nnBachelors Degree Required
n nWork Experience:
nnLess than 1 year Relevant experience. Required
n nLicenses:
nnnLicensed Physical Therapist Upon Hire Required and
n
nnBasic Life Support Upon Hire Required
n
n nCourses and Training:
nnGraduate of an accredited program in physical therapy. Upon Hire Required
