Concierge Home Care | Orange Park, FL 32073
Modified Full-Time | Schedule Flexibility | 4 Days a Week
Earn $70+++ per visit + Medicare differential pay
4 Days A Week (Thursday-Sunday)

Coverage Area: Marianna, FL / Jackson County, FL
(Territories designed to minimize drive time with clustered scheduling)
Compensation & Earning Potential
Negotiable pay-per-visit model: $70+ per visit + Medicare differential pay

Your Role as a Home Health Physical Therapist Evaluate patients and develop individualized treatment plans
Provide therapeutic exercises and mobility training
Educate patients and caregivers on safety and recovery
Collaborate with physicians and interdisciplinary teams
Supervise Physical Therapist Assistants (PTAs)
Complete documentation using streamlined technology

Qualifications Active, unencumbered Florida Physical Therapist license
