Job description
BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a part-time Outpatient at Home opportunity serving adult and geriatric clients throughout Atlantic County, NJ in Senior Living Communities .
nThis unique role combines the clinical focus of outpatient therapy with the flexibility and personalized care of home-based visits. You'll work one-on-one with patients in senior living settings, helping them regain strength, improve mobility, and maintain independence in the comfort of their own environment.

nWhat You'll Do
nnnProvide outpatient physical therapy services in designated facilities settings
n
nnPerform comprehensive physical therapy evaluations and reassessments
n
nnAssess mobility, strength, balance, gait, transfers, endurance, and overall safety
n
nnDevelop individualized treatment plans designed to maximize function and independence
n
nnImplement therapeutic exercise programs, gait training, balance training, and other evidence-based interventions
n
nnCollaborate with nurses and other healthcare professionals to coordinate patient care
n
nnEducate patients and caregivers on exercise programs, fall prevention, safety strategies, and disease management
n
nnDocument patient care using a user-friendly electronic medical record system
n

n nQualifications
nnnCurrent New Jersey Physical Therapist (PT) license
n
nnGraduation from a program approved by the American Physical Therapy Association (APTA) or the Committee on Allied Health Education and Accreditation of the American Medical Association
n
nnAbility to work independently and manage time effectively
n
nnStrong interpersonal, organizational, and problem-solving skills
n
nnSolid computer skills; prior experience with electronic medical records (EMR) preferred

nBase Pay:
n$48.00 - $53.00 per hour , depending upon qualifications and subject to negotiation.
