n nHow you'll make an impact in this role
nnnDevelop, implement, and regularly reassess individualized, patient-centered plans of care in collaboration with patients and families.
n
nnDeliver evidence-based interventions across complex diagnoses, including neurological and orthopedic conditions.
n
nnCollaborate with the interdisciplinary team to coordinate care, optimize outcomes, and support safe, effective discharge planning.
n
nnUtilize innovative rehabilitation technologies and best practices to maximize functional recovery and independence.
n
nnMinimum weekend or evening and holiday requirement.
n
n nWhat minimum requirements you'll need
nLicensure / Certification / Registration:
nnnBLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
n
nnPhysical Therapy (PT) license credentialed from the Illinois Department of Financial and Professional Regulation prior to hire date or job transfer date required. A valid license pending letter will be accepted until license is issued.
n
n nEducation:
nnBachelor's degree required. Master's OR Doctorate degree preferred.
n nAdditional Preferences:
nn1 year of Outpatient care experience.
