nBrookhaven Health Care Facility is looking to add a Physical Therapist (PT) to its rehabilitation team. In this role, you'll evaluate residents, develop individualized treatment plans, and provide skilled therapy services that help residents improve mobility, strength, balance, and overall function. nRESPONSIBILITIES:
nnnEvaluate residents and develop individualized physical therapy treatment plans
n
nnEstablish short- and long-term rehabilitation goals and monitor resident progress
n
nnProvide skilled therapy services to improve mobility, strength, balance, endurance, and functional independence
n
nnSupervise Physical Therapist Assistants (PTAs) in accordance with New York State regulations
n
nnComplete required documentation, evaluations, care plans, and billing records accurately and timely
n
nnCollaborate with physicians, nursing staff, families, and interdisciplinary team members to support resident outcomes
n
nnProvide resident and family education regarding therapy goals, adaptive equipment, and home exercise programs
n
nnRecommend and coordinate durable medical equipment (DME) and discharge planning needs
n
nnParticipate in care conferences, staff education, orientation, and quality improvement initiatives
n
nnMaintain therapy equipment, supplies, and treatment areas
n
nnPerform additional duties as assigned
n
n nREQUIREMENTS:
nnnMaster's or Doctorate Degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association
n
nnValid New York State Physical Therapist License required
n
nnNewly licensed Physical Therapists encouraged to apply
n
nnSkilled nursing facility experience preferred
n
nnStrong clinical, documentation, communication, and problem-solving skills
n
nnAbility to work collaboratively within an interdisciplinary healthcare team
n
