nMekks Home Health Care LLC is seeking a compassionate and skilled Physical Therapist (PT) to join our growing home health care team. The ideal candidate will provide high-quality, patient-centered care to individuals in their homes, helping them regain mobility, improve physical function, and enhance their overall quality of life.
nKey Responsibilities
nConduct comprehensive physical therapy evaluations in patients’ homes
nDevelop individualized treatment plans based on patient needs and goals
nImplement therapeutic exercises, manual therapy, and mobility training
nMonitor patient progress and adjust treatment plans as needed
nEducate patients and family members on exercises, safety, and injury prevention
nMaintain accurate and timely clinical documentation in compliance with state and federal regulations
nCollaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care
nEnsure compliance with all home health care standards and company policies
nQualifications & Requirements
nDoctor of Physical Therapy (DPT) or equivalent degree from an accredited program
nActive Massachusetts Physical Therapist License
nMinimum of 1 year experience (home health experience preferred but not required)
nStrong clinical assessment and patient care skills
nExcellent communication and interpersonal skills
nAbility to work independently and manage time effectively
nReliable transportation and valid driver’s license
nPreferred Skills
nExperience with geriatric patients and chronic conditions
nFamiliarity with electronic medical records (EMR) systems
nStrong organizational and documentation skills
nWork Schedule
nFlexible part-time hours based on patient needs and therapist availability
